Telephone Triage - With effect from the 8th June 2016
In order to make better use of our same day appointments you will first need a Telephone Consultation with a Clinician.
This also applies to patients that walk-in.
- Following the Telephone Consultation you will be offered a face to face appointment if appropriate.
- If a face to face appointment is not required additional services may be signposted.
Please note that Telephone Triage is not for:
- Fit (Sick) notes
- Existing Problems
- Non Urgent Problems
All Prescription and Fit Note requests will be processed by our Admin Team in line with our published turn around times.
Existing and non-urgent problems will require a routine pre-bookable appointment.
Please note the surgery does not provide letters for children that have missed exams.
The BMA has sought and received confirmation from the Office of the Qualifications and Examinations regulator that Awarding Organisations make no requirement for pupils to obtain a medical certificate in support of their application for special consideration.
All patients have a named GP.
This GP is responsible for the co-ordination of all appropriate services required under the contract and ensuring they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).
The GP you next see will confirm who is your named GP, but please don't hesitate to ask.
If you have a preference to a named GP please let us know and we will try to accommodate your request.
Our Patient Online Service
Our Online Service enable you to book and cancel appointments, view your appointment history, order your repeat medication, update your contact details and view your summary clinical record and detailed coded record. Before using the service you must be registered. In most cases, you will need to do this at the surgery and you will need proof of ID if you are registering for access to your medical record.
Crown Heights Medical Centre - Patient Reference Group/Participation Group Report
Crown Heights Medical Centre has undertaken two surveys in 2014/15 and secured the views of our patient participation group and patient reference group to determine how investments may be made to support the practice during 2015/16. A copy of the report is available.
